CARE COORDINATION
Mom wants to come back home after she gets out of the hospital, but she
refuses to let anyone come in and help. Dad's doctors don't seem to know
what the others are doing. Dad isn't eating his meals at the nursing home,
and the staff doesn't seem to know what medications he is taking.
Too often we hear our clients or their families express their utter
frustration over trying to figure out how to fulfill the multitude of
requirements specified by doctors and other care providers, pharmacists,
and facilities, while trying to make sure their loved ones are getting both
what they need and what they want. It can become terribly difficult to do
the right thing when feeling pulled in every direction. They often look to
us to help them navigate these tricky issues and find solutions. Our
Geriatric Care Coordinator is happy to step in and help you determine not just
what they need, but who should provide it and where.
A Care Coordinator is a geriatric professional with a background in social
work, nursing or mental health. Our Life Care Planning clients meet with
our Geriatric Care Coordinator immediately after we start working together and
makes a complete assessment of the client's specific needs. Then, she
creates the plan that fills those needs. She monitors your loved one's care
closely and makes or recommends appropriate changes to the care plan as
needed. She also serves as the front line advocate seeing to it that your
loved one always receives the best care regardless of whether it is at home,
in an assisted living facility or in a nursing home.
Our Geriatric Care Coordinator is also an expert who knows our local nursing
homes, assisted living and other care facilities, and will advise you on the
quality, availability and costs of long term care in Riverside and
surrounding areas.
